Never Seen that Before is a Challenges Achievement which requires killing at least one of the randomly spawning Unique Monsters (with purple names and additional titles).
Until patch 1.0.3b, Saha the Slasher and Gart the Mad were bugged and could not be found in-game. This was fixed in patch 1.0.4.
