The Sand Maggot Lair is a network of tunnels dug by Sand Maggots. The narrowness and the crookedness of the tunnels is unique in the game and changes how the dungeon works. Players cannot walk through each other, so one person has to run first. In the same way, hirelings cannot pass through the player, while the player can pass through them. Necromancers, or anyone summoning, had a hard time managing their minions. On the other end, the narrow corridors also negates some of the danger presented by the Scarab Demons.
The main boss of the hole is a giant Maggot Queen named Coldworm the Burrower, who remains stagnant within its cavern constantly creating new maggots in the colony. There is a chest present, but it only drops gold and the Staff of Kings when the quest is active.
