The Black Abyss is a mysterious place briefly mentioned in Diablo II by Tyrael, on the Arreat Summit (using the same text as the in-game dialogue of Tyrael) and in the Book of Cain. In the Book of Cain it was confirmed that this place is where Diablo and possibly other powerful demons end up when they're killed, and that demons can be summoned from this realm. It is a complete different realm than heaven and hell, where supposedly the entities inside cannot escape nor influence the other realms
Note that the Black Abyss is not the same place as The Void which is where Inarius imprisoned Rathma and Lilith, and where Lilith remains trapped to this day.
